REFERRAL PROCESS

In order to book an appointment we require a referral from a medical doctor or nurse practitioner. This also applies for patients who have not been seen in the last year (365 days). Referrals may be faxed to 709-726-6634.

Our services are covered by MCP.

When you speak to your provider please ask to have your email address included on your referral. All of our referrals are reviewed for appropriateness. If the referral has been accepted by the office we will email an invitation to book online.

While we do have a referral form some providers find helpful, it is not required when making referrals. This form is also available in MedAccess.

Referrals are normally processed within 3-14 business days.

If no email address is provided, or if you do not have an email address please contact the office in order to book an appointment. Please allow at least 3 business days before you call.

If you find a booking invitation in your email/spam folder that has expired, please email us and we will send you a new link. Invitations come from no-reply@cognisantmd.com
